Please note: Submitting this form completes your initial CVAC event intake, but it does not confirm your registration.

When you click "Submit" below, you will be automatically routed to the Meals on Wheels website to make your required $50 donation. (Note: The $50 donation is per doubles team, but you are more than welcome to donate more!)

To finalize your registration on the Meals on Wheels website:

Step 1: Fill out the necessary information on their donation form.
Step 2: Under "My gift is to be used for", select "Pickleball Tournament".
Step 3: Under "Donation Amount", select $50 (100% of the proceeds go to Meals on Wheels!).
Step 4: Fill out the remainder of the billing details and click submit.

Registration Confirmation: Once you have completed the donation and received your confirmation email directly from Meals on Wheels, you are officially registered for the tournament!
